- headers and footers implemented and formatted
- GroupOn for all values, i.e. Day, Week, Hour, etc.
- treat a group with no expression as a single implicit group
- skip empty group expressions when building the ORDER BY clause
- fix nested-group detection: an outer group's change now
always forces every inner group to break too, even when the
inner group's own value happens to coincide across the boundary
- add nestedGroupBoundary CppunitTest + nested_group_boundary.odb
