Noel Grandin 3af500580b loplugin:salcall fix functions 
since cdecl is the default calling convention on Windows for
such functions, the annotation is redundant.

Noel Grandin 3f47ffcf58 OModule::onFirstClient is unused 
also move the MutexGuard in revokeClient. No need to hold it while doing
an osl_atomic_increment. Save it until we actually need to call
onLastClient.
