Noel Grandin 0f874472c6 size some stringbuffer to prevent re-alloc 
found by the simple expidient of putting asserts in
the resize routine. Where an explicit const size is used,
I started with 32 and kept doubling until that site
did not need resizing anymore.

Gabor Kelemen 2f04460c83 tdf#42949 Fix IWYU warnings in include/ 
Recheck directories under include/ and below vcl in the
module hierarchy.

New IWYU and recent developments in f-u-i helped to identify
some non self contained files, those were fixed too.

Also some new removal proposals are dealt with here.
